Android mileage tracking suggestion/form?

So, for those what haven't heard, I drive for a living, specifically as an IC (independent contractor) courier. I'm kinda old school in that I track my mileage on paper, but I'd like to go electronic.

There's an app for that...
Yeah, but it's usually over kill, incorporating various tax features and other stuff, I just want to replace my notebook. Heck, I could use a #Google form feeding a spreadsheet, and that'd work great... Except I'd like to keep Google out of my life.

So, anyone know of a forms type solution for Android? I need client/app, 'route', start mileage, end mileage, pay on the sheet. Optional stuff is start time, load time, work time, and pay per mile (PPM), and output to a spread sheet is prolly best.

Anyone got any ideas?
