World Health Organization Guideline for Lower Back Pain: Yes to Acupuncture; No to Most Drugs

On Dec. 7, 2023, the World Health Organization (WHO) released its “Guideline for Non-Surgical Management of Chronic Primary Low Back Pain in Adults in Primary and Community Care Settings.” The purpose of the 244-page document is to “provide evidence-based recommendations on nonsurgical interventions for chronic primary LBP in adults” that will improve outcomes. Acupuncture is among the interventions the guideline recommends for all adults, including older adults.

The guideline reviews both the benefits and the harms of nonsurgical interventions in the management of chronic primary Low Back Pain. In addition to acupuncture, the guideline is also in favor of structured exercise, massage, spinal manipulation, NSAIDs and topical cayenne pepper (Capsicum frutescens).

Acetaminophen, benzodiazepines and cannabis-related pharmaceutical preparations received “no recommendation,” while listing potential harmful effects including “cardiovascular, renal and gastrointestinal harms and increased mortality risk” for acetaminophen; “potential harms including memory impairment, misuse, overdose deaths from respiratory depression, somnolence, fatigue and light-headedness potentially leading to falls” for benzodiazepines; and “evidence of possible adverse events, including harms associated with its nonmedicinal use” for cannabis-related pharmaceutical preparations.

There was also a significant pretreatment to post-treatment reduction in fear-conditioned extinction, which was assessed by a fear-potentiated startle response -- a sudden blast of air paired with a certain image, such as a blue box -- in the group receiving verum acupuncture versus the sham group, and a significant correlation (r=0.31) between symptom reduction and fear extinction.

Just because we practice traditional Chinese medicine, doesn't mean we condone animal cruelty, so we are happy that African Heads of State and Government recommend a continent-wide ban on the slaughter of donkeys for their skin.
There are many alternatives to animal products, including different herbal medicines, foods, qi gong, moxa and even acupuncture. Chinese medicine has a very long and very noble history, and has always adapted to the needs and customs of the times. It can do without products that are not ethical to use in the 21st century.

Moxa is an important part of Acupuncture practice. Acupuncturists will often ask their patients to apply moxa at home between treatments. University Hospitals pilot program offering #acupuncture for #fertility

It's estimated between 10%-15% of couples experience fertility issues. The reasons can cause stress and certainly impact well-being.
A year ago, University Hospitals Fertility Center surveyed patients about what they felt were their unmet psychosocial needs. The survey found that 80% of survey participants wanted to use acupuncture in their journey and 70% desired health, nutrition, stress, exercise, and supplement guidance.

"There's actually a lot of research around specifically acupuncture and IVF treatment. It's been studied for, I want to say, over 15 years now. And we continue to see success with adding acupuncture to IVF protocols. I take it a step further and also add the lifestyle medicine piece to the fertility journey, because I think it's important to focus on nutrition, stress management, sleep, exercise because we see a correlation to helping fertility as well in the research. So our our Vitamix group is really focused on learning all of those tools and then having the support of a health coach to implement those healthy habit changes that are often hard to do on your own. And incorporating that with other modalities like acupuncture, massage, chiropractic that we offer here as well," said Christine Kaiser, DACM, LAc, LCH, FABORM who is the Connor Endowed Director of Reproductive Well-being and Clinical Manager of Acupuncture at University Hospitals Connor Whole Health.
